How to Enroll


Once accepted as a patient click HERE and enter your information and your preferred payment method. The DPC fee will be automatically deducted from your bank account or charged to your credit card, either monthly or annually. If using the website is problematic for you, please complete this form and return to us.

What’s the next step after I enroll?
Once you enroll, you will receive an email invitation from our new secure patient portal, called “Elation Passport”. Please be sure to “ACCEPT” this invitation from Elation Passport by entering the code sent to you by text, so you can communicate via our secure email communication. By doing this, you will be registering for the patient portal. Please let us know if you have any problems with this portal, or if you do not get a response within a few days, as the practice depends on open, easy, and reliable communication with you via the secure portal.

Please Note: Secure Electronic Health Records portal communications are for non-urgent matters, and can take 2-3 business days for a response. As always, if you are sick or have an urgent need , always CALL THE OFFICE instead of messaging  AND call 911 if you have a medical emergency, chest pain, trouble breathing,, bleeding, severe pain, or another life threatening emergency.

What if I have Medicare?
Medicare patients are not eligible for Direct Primary Care. However, you can receive the same individualized primary health care as our DPC members with the payment of an annual fee of $500 ($42/month) to compensate for services we provide to all patients that are not covered by Medicare. This fee is for non-covered services including, direct access to the doctors phone, email and telephone visits, and phone calls with you and/or specialists/hospital physicians. This offers  advocacy for your best care, whenever possible when you are hospitalized or need immediate specialty care.

If you choose to join our practice as a Medicare patient please fill out the application and choose the 65 and above category. The annual fee is $500, and will be charged to your credit/debit card at the start of each 12 month enrollment period.

What if I am not sure if I want to become a DPC plan member, but I still want care?
You will still be able to obtain medical care from this office on an out-of-pocket-fee-for-time basis, once you are a member of our practice. The “cash-pay” rate is $50 per 15 minute of the doctor’s time, by phone, email, or in person, plus the cost of any medical supplies used in your care.  If you choose this option, please apply to be a patient and inform us that you would like to be seen on a cash pay basis.

*Payment for non-covered Medicare services provided by this practice is required upfront.
