How to apply & register for an appointment 

Once accepted for a medical consultation or an immigration medical exam please click HERE and enter your information and your preferred payment method. Please note: 1. a credit card number is necessary to activate a chart and request an appointment via our registration system and 2. you will NOT BE CHARGED anything until you have seen the doctor.  Thank you for your understanding.  We aim to be cost transparent – and will do our best to advise you in advance of any charge that you will incur in seeing Dr. Scriven. Laboratory test pricing have been negotiated to the lowest we can receive by Max Health Office. Our policy is to pass those savings on to you, as a consumer. 

What’s the next step after I register for a medical consultation (or immigration medical exam application)?
Once you register, you will receive an email invitation from our new secure patient portal, called “Elation Passport”. Please be sure to “ACCEPT” this invitation from Elation Passport by entering the code sent to you by text, so you can communicate directly with the doctor, via our secure email communication. By doing this, you will be registering for the patient portal. Please let us know if you have any problems with this portal, or if you do not get a response within a few days, as the consultations and you receiving your own lab results for your records,  depends on open, easy, and reliable communication with you via the secure portal.

Please Note: Secure Electronic Health Records portal communications are for non-urgent matters, and can take 2-3 business days for a response. As always, if you are sick or have an urgent need , always CALL THE OFFICE instead of messaging  AND call 911 if you have a medical emergency, chest pain, trouble breathing,, bleeding, severe pain, or another life threatening emergency.

What if I have Medicare or Maine Care?
Unfortunately we do not directly participate in Maine Care or Medicare.  However, you can ask us to submit any lab orders with insurance information, should you desire that the laboratory try to receive reimbursement from your insurance.  However our medical visits are not covered by insurance, nor is any immigration medical exam.  If you have financial hardship, please contact the office or the Good Medicine Collective regarding sliding scale or scholarship – this is reserved for those who really can’t pay due to food, shelter, heat type needs.

Medical Consults details: Dr. Scriven will spend at least one hour with you to help sort out your non- urgent medical or mental health concerns, and provide initial treatment if needed. The goal of the medical consults Dr. Scriven offers are often to help properly identify symptom complexes with you, possible diagnoses, and who, in the broader medical and integrative community, can help you. After such consultations, you are referred to your own PCP, informed and often encouraged, with an idea of desired next steps, needed referrals, or testing recommendations. If you don’t already have a PCP, we can provide referrals to direct primary care doctors in the area, and/ or specialists, that most likely would be helpful to you, given your symptoms or disease. The goal of Dr. Scriven’s visits with patients is to help you identify medical problems, and the steps you can take to make you better, including initial ( but not ongoing ) treatment and support in order to obtain referral to the services you need.

Costs generally are $50 per 15 minutes of doctor’s time in face to face visit AND also with any additional doctor work necessary to prescribe, review records, and make specialty or primary care referrals for ongoing care, and records completion and forwarding. There is a sliding scale fee schedule available upon request.

Any recommended lab tests patient agrees to will be sent to a laboratory with best possible pricing available and patient in agreeing to these costs, will be responsible for the laboratory charges. They are not included in visit fees.
